This general term tends to apply to the lesser toes, those other than the "big" toe. It covers positional and alignment problems with the toes, with the associated painful problems which can develop due to the digital malalignment. There is usually some form of joint contracture causing the poor alignment, and this contracture can be either flexible, semiflexible, or rigid. Additionally, rotational deformities can also be a contributing factor. Although these deformatities are often not painful or symptomatic, the abnormal positioning can result in abnormal shoe pressure, callus formation, or even ulcer development. Conservative care usually requires some change in the types of shoes the patient wears, periodic trimming of calluses and corns which can develop, or possibly some form of insert in the shoes. For persistent cases or those not responding to conservative care, surgery is usually indicated.
